SYNC

SYNC is a robust platform that facilitates client referral and service coordination for individuals and families within the community. It is a HIPAA-compliant tool that enables secure data-sharing between multiple agencies. The platform was created in 2023 through collaborative efforts of the AACN, Kronkosky Charitable Foundation, United Way of San Antonio and Bexar County, ConnectAbility, Autism Lifeline Links, and the City of San Antonio. SYNC empowers the AACN and other community-based organizations to capture social determinants of health, identify client needs and gaps in services, collect and analyze data, and reduce duplication of efforts. By leveraging SYNC, our community will continue to benefit from enhanced care and services. We are excited to share this innovative approach to improve social and community health outcomes.
