SYNC
SYNC is a powerful community care coordination platform designed to streamline client referrals and enhance service delivery for individuals and families across our region. As a HIPAA-compliant solution, SYNC enables secure information sharing among partner agencies, fostering greater collaboration and continuity of care.
Launched in 2023 through a collaborative partnership between the Alamo Area Community Network (AACN), Kronkosky Charitable Foundation, United Way of San Antonio and Bexar County, ConnectAbility, Autism Lifeline Links, and the City of San Antonio, SYNC was created to strengthen connections between organizations and the communities they serve.
By leveraging SYNC, community-based organizations can better identify and address social determinants of health, assess client needs, uncover service gaps, track outcomes, and reduce duplication of efforts. The platform provides valuable data insights that support informed decision-making and more effective resource allocation.
Together, we are building a more connected and responsive network of care. Through innovation, collaboration, and shared accountability, SYNC helps improve access to services and drive better social and community health outcomes for all.
